About this skill

The principle of treating according to the person

R — Original Text (Reading)

Those who are mature in age and strong in build, with abundant blood and qi, and firm skin... needle deeply and retain the needle. The thin person has thin skin and pale complexion, with flesh tightly drawn, thin lips, and speaks softly... needle shallowly and quickly. Infants have fragile flesh, little blood, and weak qi... use a fine needle, needle shallowly, and withdraw the needle quickly. Those in ancient times who were skilled in using acupuncture and moxibustion treated people according to their five states.

— Huangdi/Qibo, Chapter 38 "Ni Shun Fei Shou" and Chapter 72 "Tong Tian"

I — Methodological Framework (Interpretation)

Treating according to the person is a decision-making framework that adjusts plan parameters according to individual differences.

  1. Identify dimensions of difference: Which individual differences affect the plan's effectiveness? The Lingshu focuses on: constitution (fat/thin/strong/weak), age (old/prime/young/infant), personality (five states), and social status (commoner/noble). Different dimensions correspond to different parameter adjustments.
  1. Parameters vary by person: In the same plan framework, key parameters (depth, speed, intensity, frequency) must vary by person. For the fat person, needle deeply and retain; for the thin person, needle shallowly and quickly; for infants, the shallowest and fastest—the direction is the same but the parameters differ.

  2. Matching rather than standardization: "To act according to what is suitable is the truth"—the plan that best matches the individual's characteristics is the best, not the "standard plan."

  3. Observe first, then adjust: One must first identify the individual's characteristics through "governing the outside to infer the inside," and then adjust accordingly; one cannot rely on subjective assumptions.

A1 — Application in the Text (Past Application)

Case 1: Parameter differences for fat, thin, strong, and weak

  • Problem: For the same acupuncture, what different parameters are needed for different constitutions?
  • Use of the methodology: Fat person (turbid blood and sluggish qi) → needle deeply and retain, increasing the number of needles; thin person (clear blood and slippery qi) → needle shallowly and quickly; ordinary person → do not lose the standard number; strong person (heavy) → needle deeply and retain; strong person (vigorous) → needle shallowly and quickly; infant → needle shallowly and withdraw quickly, twice a day is acceptable.
  • Conclusion: Six constitutions, six sets of parameters. Parameter differences come from the "slippery/sluggish" and "strong/weak" qualities of blood and qi.
  • Result: Each constitution received the most matching plan, avoiding the ineffectiveness or harm of a "one-size-fits-all" approach.

Case 2: The difference between nobles and commoners

  • Problem: For patients of different social classes, is the needling method different?
  • Use of the methodology: "How can the flavors of rich fare and coarse greens be the same?" Nobles and high officials have delicate bodies and bold, slippery, and smooth blood and qi, requiring "gentle and slow needling." Commoners and ordinary men have sunken and sluggish blood and qi, requiring "deep needling and retention."
  • Conclusion: Lifestyle (diet/exercise) shapes constitution, and constitution determines plan parameters.
  • Result: Fairness is not based on the "same plan" but on the "most matching plan."

A2 — Trigger Scenarios (Future Trigger) ★

  1. Standardized plans fail: The same plan is effective for A but ineffective for B, requiring individualization.
  2. Parameter adjustment: The plan framework is correct but the effect is not ideal, requiring fine-tuning of parameters.
  3. Team management: Need to assign different tasks according to different members' abilities and styles.

Language signals

  • "Why does the same method work for others but not for me?"
  • "One-size-fits-all solutions don't work"
  • "Different people need different treatment"
  • "How to adjust according to the actual situation"

E — Executable Steps (Execution)

  1. Identify the difference dimensions that affect effectiveness

    • List individual characteristics that may affect the effectiveness of the solution (ability/experience/preference/resources).
    • Completion criteria: Identify at least 2 key difference dimensions.
  2. Establish a difference-parameter mapping

    • How does each difference dimension affect the key parameters of the solution (depth/speed/intensity/frequency)?
    • Completion criteria: A clear mapping table, differences → parameter adjustment direction.
  3. Customize the solution

    • Based on the mapping relationship, adjust the solution parameters and generate a customized version.
    • Completion criteria: At least 1 key parameter is adjusted, and the reason for the adjustment is explained.

B — Boundary ★

Do not use in the following situations

  • Standardization takes priority: Some scenarios (such as safety regulations) do indeed require standardization; personalization may cause safety risks.

Failure modes

  • Over-personalization: Everyone being different does not mean everyone needs a completely different solution; the key is to identifyaffect effectivenessthe difference dimensions.
  • Stereotyping: Directly equating appearance/identity with ability, rather than judging through actual observation.
