In all cases of using needles: if deficient, tonify it; if full, drain it; if stagnant and accumulated, remove it; if pathogenic factors prevail, reduce them. Slowly insert and quickly withdraw, then it becomes replete; quickly insert and slowly withdraw, then it becomes deficient. When speaking of repletion and depletion, it is as if present and as if absent. If the physical constitution and qi are insufficient while the disease qi is in excess, drain it urgently; if the physical constitution and qi are in excess while the disease qi is insufficient, tonify it urgently.

I — Methodological Skeleton (Interpretation)
Deficiency-excess tonify-drain is a binary decision framework used to judge the direction of an imbalance and choose a corrective strategy.
Its core operation is divided into three layers:
- Judge deficiency and excess: Judge whether the problem is "insufficient" (deficiency — lacking, weak, not enough) or "excessive" (excess — surplus, overabundant, accumulated). This is a directional judgment — if deficient, tonify (increase); if excess, drain (reduce). The wrong direction is worse than no intervention.
-
Tonifying and draining are different: Tonifying and draining are not simply "adding and subtracting." Tonifying is "slowly insert and quickly withdraw, then it becomes replete" — accumulate slowly and then obtain results quickly; draining is "quickly insert and slowly withdraw, then it becomes deficient" — remove quickly and then recover slowly. The operational rhythm is different.
-
Comprehensive judgment from multiple signals: When the external manifestation (physical constitution and qi) and the deficiency-excess of the problem itself (disease qi) are inconsistent, "drain it urgently" or "tonify it urgently" — take the deficiency-excess of the problem itself as the priority basis, and the external manifestation as reference.
-
Direction determines everything: "If deficient and you drain it, then the channels and vessels become empty, and blood and qi are exhausted and withered; if full and you tonify it, then yin and yang overflow in all four directions" — the wrong direction is more fatal than insufficient force.
A1 — Application in the Text (Past Application)
Case 1: Inconsistency between the deficiency-excess of physical constitution and qi and disease qi
- Problem: How to handle the situation where the external constitution (physical constitution and qi) and the actual disease condition (disease qi) are inconsistent in deficiency-excess?
- Use of the methodology: "If the physical constitution and qi are insufficient while the disease qi is in excess, this is the prevalence of pathogenic factors; drain it urgently" — although the constitution is weak, the pathogenic factors are strong, so at this time one should instead drain (eliminate pathogenic factors). "If the physical constitution and qi are in excess while the disease qi is insufficient, tonify it urgently" — although the constitution is strong, the upright qi is insufficient, so at this time one should instead tonify (support upright qi).
- Conclusion: One cannot simply look at "if the body is strong, drain; if the body is weak, tonify," but must look at "the direction of deficiency-excess of the problem itself."
- Result: Avoided the dogmatic error of "if the body is weak, tonify," and achieved precise directional decision-making.
Case 2: Conservative strategy when both yin and yang are insufficient
- Problem: When the system is comprehensively weak ("both yin and yang qi are insufficient"), neither excess nor overabundant, what should be done?
- Use of the methodology: "It cannot be needled; needling it will make the insufficiency worse" — one cannot drain or strongly tonify; "one may treat with sweet medicines" — use gentle regulation instead of forceful tonifying and draining.
- Conclusion: When the system is comprehensively weak, forceful intervention in any direction is dangerous, and gentle regulation is needed.
- Result: "If the insufficiency is worsened, then both yin and yang are exhausted" — avoided comprehensive collapse caused by excessive intervention.
A2 — Trigger Scenarios (Future Trigger) ★
In what situations will users need this skill?
- Directional confusion: When facing imbalance, one does not know "whether to add or subtract" or "whether to invest or withdraw".
- The more it is handled, the worse it becomes: After intervention, the problem worsens instead; the direction of tonification or reduction may be wrong.
- Polarized imbalance: The system clearly shows one side too strong and one side too weak, and the direction of correction must be determined.
Verbal signals
- "I don't know whether to strengthen or cut back"
- "The more I tonify, the more deficient it becomes / the more I reduce, the more chaotic it becomes"
- "One side is in excess while the other is insufficient"
- "The system swings between two extremes"
Distinction from adjacent skills
- With
qi-regulationthe difference: regulating qi to treat the root is "whyto intervene in function", while deficiency-excess tonification-reduction is "howto intervene (tonify or reduce)". - With
root-cause-prioritythe difference: deficiency-excess tonification-reduction resolves "direction" (tonify/reduce), while branch-root and root-knot resolves "order" (which to treat first).
E — Executable steps (Execution)
-
List the two poles of the imbalance
- Clarify the opposing sides in the system (e.g., offense vs defense, output vs quality, speed vs safety).
- Completion criterion: The two poles are clearly defined and do not mix different dimensions.
-
Determine the deficiency-excess direction
- Assess whether each side is "in excess" or "insufficient". Distinguish "absolute excess" from "relative excess" (one side is normal but the other is insufficient, causing imbalance).
- Completion criterion: Give a clear judgment—"X is deficient and Y is excess" or "X is normal and Y is insufficient".
- Stop condition: If both sides are deficient (overall insufficiency), skip to the conservative strategy in step 4.
-
Determine the tonification-reduction direction
- If excess, reduce (cut back the excessive side); if deficient, tonify (augment the insufficient side). Prioritize correcting the insufficient side, because "restoring the deficient" is more fundamental than "reducing the excess".
- Completion criterion: Give a clear "tonify X / reduce Y" direction and at least 1 concrete action.
-
Note boundary cases
- Overall weakness: Do not strongly tonify or reduce; use gentle regulation instead.
- Surface contradiction: When external manifestations are inconsistent with the actual deficiency-excess state, base the judgment on the actual problem.
- Completion criterion: Identify and mark boundary cases.
B — Boundary ★
Do not use this skill in the following situations
- No opposing side: If the problem has only a single dimension and no opposing side for comparison, the deficiency-excess framework cannot be unfolded.
- Pure balance assessment: The user only wants to know "whether it is balanced" and does not need to decide the direction of correction.
Failure modes the author warns about in the book
- Direction reversed: Treating deficiency as excess and reducing it, or treating excess as deficiency and tonifying it—the consequences are more serious than no intervention.
- Strong attack when both yin and yang are deficient: When the system is comprehensively weak, strong reduction or tonification will lead to "exhaustion of both yin and yang".
The author's blind spots / limitations of the era
- The deficiency-excess in the Lingshu is a qualitative judgment, while modern decision-making may require quantitative data as an aid. The deficiency-excess framework should serve as an auxiliary perspective for judging direction and does not exclude data-driven precise analysis.
