Israeli Fertility Treatment Guide

Legal notice

This is a free information tool operated by an artificial-intelligence model. It explains how fertility treatment is funded in Israel: health-basket entitlements under the National Health Insurance Law, supplementary-insurance (SHABAN) benefits, the published rules on egg freezing, sperm donation and egg donation, and what the law and official directives provide on surrogacy, posthumous sperm use and work rights. All of its output is produced automatically, without the involvement, review or approval of a physician, fertility specialist, social worker or lawyer. The output is not a diagnosis, not a treatment recommendation, not a medical opinion and not legal advice, but general information about the published rules only: it involves no examination of and no meeting with a patient, does not consider your medical indication or personal circumstances, and does not determine eligibility, coverage or the chance that a treatment will succeed. Eligibility is decided by your kupat cholim, its fertility committee or the competent authority. An artificial-intelligence model may err, omit data or present a wrong conclusion, so verify every figure with your kupah, with the fertility unit or with the competent authority.

Do not rely on the output to decide on treatment or medication, to sign a surrogacy agreement, to apply to the Family Court or in any other legal proceeding, or to file a complaint or claim. This tool is not a substitute for advice that takes into account the particular data and needs of each person; before any medical decision consult your treating physician, and before taking legal action or signing a document consult a lawyer. Any use of the output is at the user's sole responsibility.

Instructions

Step 1: Israel's Fertility Treatment Landscape

Israel is a world leader in IVF (in vitro fertilization,הפריה חוץ גופית) per capita.

The governing law is the National Health Insurance Law (National Health Insurance Law), and an annual committee (health basket committee) decides what enters the basket. Funding covers the first 2 living children, with the woman's own eggs from 18 until her 45th birthday and with a donated egg until her 54th birthday. There is no general cycle cap, but a limit applies from age 42 (Step 2). Treatment happens in licensed fertility units, mostly in public hospitals.

Types of fertility treatments covered:

Treatment Hebrew Name Covered by Basic Insurance
IVF In vitro fertilization (IVF) Yes, up to 2 living children
IUI and ovulation induction Intrauterine insemination (IUI), ovulation induction Yes
ICSI Intracytoplasmic sperm injection (ICSI) Yes
Egg freezing (medical) Egg freezing (medical reasons) Yes, per indication (Step 5)
Egg freezing (elective) Elective egg freezing No (Step 5)
Sperm donation Sperm donation Procedure yes, the sperm vial NO (Step 5)
Egg donation (recipient) Egg donation Yes, minus a statutory fee (Step 5)
PGT for BRCA1/BRCA2 and Lynch carriers PGT for BRCA / Lynch carriers Yes, capped at 2 pregnancies ending in birth

Step 2: Health Basket Coverage for Fertility

The health basket (sal briut,סל בריאות) defines what is covered under the National Health Insurance Law.

Coverage Rule Details
Eligibility All Israeli residents registered with a kupat cholim
Live birth limit Funding runs until 2 living children from the current relationship. The statute counts children, not pregnancies. Twins from one funded birth exhaust it: במקרה שנולדו תאומים, תיחשב האישה כמי שמיצתה את זכותה
Age, own eggs 18 to 45, עד יום הולדת 45. A commitment issued at 44 must be used before the official 45th birthday
Age, donated egg Funded treatment with a donated egg runs until the 54th birthday. Turning 45 ends the own-egg track, not fertility coverage as a whole
Cycle rule from 42 Per MoH Circular 6/2014, a woman who has turned 42 may not perform more than 3 consecutive IVF cycles that did not reach embryo transfer. Any transfer, even in the third cycle, restarts the count
Mandatory team review After 4 consecutive cycles with no transfer, or 8 IVF cycles with no clinical pregnancy, the treating team (with a social worker) must review the causes and the plan going forward
Single women and female couples Single women (em yechidanit) get the same IVF and donor-insemination coverage as couples, and each partner in a female couple qualifies in her own right. For a child born from anonymous sperm donation, the non-biological mother is recognised as a parent through a judicial parenthood order (tzav horut psikati) from the Family Court, without an adoption process
Reciprocal IVF NOT permitted in Israel. One partner's egg may be fertilized here, but not transferred into her partner's uterus in Israel
Relationship change For a couple, children from a previous marriage do not count: the entitlement is to a common child. The single-parent track is published for a woman without children, so a single woman who already has children should confirm with her kupah
Which kupah pays The kupah where the WOMAN is insured funds treatment, even when the infertility is male-factor. Only treatment of the man's sperm is funded by his own kupah

What the basket covers in each IVF cycle: hormonal stimulation medications, monitoring ultrasounds and blood tests, egg retrieval with anesthesia, laboratory fertilization, embryo transfer, embryo freezing, and frozen embryo transfer (FET). What it does NOT cover: a donor sperm vial, which the patient buys herself.

The Health Basket Committee meets annually. The 2026 basket (Director General Circular 02/2026, published 08.03.2026) added PGT (preimplantation genetic testing) as part of IVF for carriers of high-cancer-risk mutations (BRCA1/BRCA2 and Lynch syndrome). That entitlement is capped: הזכאות לטיפול לפי פסקה זו, תוגבל לשני הריונות בלבד, שהסתיימו בלידה.

Step 3: The IVF Process Through Kupat Cholim

Route: gynecologist or family doctor referral, then a fertility specialist (fertility doctor), then approval by the kupah's fertility committee (fertility committee), then a Form 17 commitment (Form 17) to a licensed IVF unit in a public hospital. Diagnostics usually include a hormonal blood panel, ultrasound, semen analysis and HSG. references/ivf-process-detailed.md has the full walkthrough.

Choosing a clinic: each kupah has agreements with specific units (Sheba, Ichilov, Hadassah, Rambam, Soroka and Assuta among the largest). You may pick a unit other than the default, but verify coverage first, and wait times differ. Donor insemination may NOT be performed in a private clinic (חל איסור לבצע הזרעה מלאכותית מתורם במרפאה פרטית).

Safety note (OHSS,גירוי יתר שחלתי): ovarian hyperstimulation syndrome is the most common serious complication of stimulation. Severe abdominal pain or swelling, rapid weight gain, vomiting, breathlessness or a sharp drop in urine output after retrieval are emergency symptoms. Contact the treating fertility unit immediately, and go to an emergency room (or call 101) if severe.

Step 4: Supplementary Insurance (SHABAN) for Additional Coverage

Supplementary health insurance (shaban,שב"ן,ביטוח משלים) extends fertility coverage beyond the basic health basket.

Kupat Cholim SHABAN Plan Names Fertility Benefits
Clalit (Clalit) Mushlam, Mushlam Platinum Extra IVF cycles, elective egg freezing, choice of doctor
Maccabi (Maccabi) Maccabi Sheli, Zahav Extended coverage, elective egg freezing, premium clinics
Meuhedet (Meuhedet) Adif, Si (Si) Extra treatments, elective egg freezing, specialist access
Leumit (Leumit) Kesef, Zahav Fertility coverage, and since 2026 elective egg freezing on Zahav

What SHABAN may add: extra IVF cycles for a third or later child, elective egg freezing (all four kupot now, on different terms, see Step 5), choice of a specific doctor, shorter waits at private or semi-private units, certain costs around egg donation and treatment abroad, and some premium medications outside the basic basket.

Important: without SHABAN, treatment for a third child or beyond is fully out-of-pocket. SHABAN fertility benefits carry a qualification period (tkufat achshara,תקופת אכשרה), 12 months on the Maccabi and Leumit elective egg-freezing benefits documented below, so enroll before you need it.

Switching kupah for a better fertility benefit: the move takes effect only on one of 6 fixed dates a year, set by when the form is filed. SHABAN seniority carries over at the same tier, but the receiving kupah may still impose qualification periods for new services, and moving to a higher tier triggers its full qualification period.

Step 5: Egg Freezing, Donation, Surrogacy, and Posthumous Use

Egg Freezing (hakpaat bitziot,הקפאת ביציות):

Aspect Details
Medical egg freezing Basket-funded for girls and women facing chemotherapy or radiation that may harm fertility, and up to age 39 for women at increased risk of premature ovarian failure (fragile-X premutation carriers, women with a disease proven to raise that risk, women before gonadotoxic treatment not due to malignancy, and women facing prophylactic ovarian surgery), per the conditions
Elective egg freezing NOT in the basic basket at any age, but permitted since 2011 for women who have turned 30 and not yet 41, at their own expense. Circular 1/2011 caps it at 6 retrievals, 25 eggs at ages 30 to 35 and 35 eggs at 36 to 40. These are Ministry of Health limits on a self-funded procedure, not private SHABAN limits
SHABAN subsidies (verify, these changed recently) All at 3,500 NIS copay per cycle. Clalit Mushlam Platinum: 30 to 37 (עד יום הולדת 38), 2 retrievals / 25 eggs at 30 to 35, 3 retrievals / 35 eggs at 36 to 37, 5 years storage included. Maccabi Sheli: 31 to 38 inclusive, up to 3 treatments or 25 eggs, from 15.03.2026. Meuhedet Si: its public page gives the copay but no age band, so confirm with Meuhedet; the MoH window ends at the 41st birthday. Leumit Zahav: 30 to 37 inclusive, up to 4 rounds. Maccabi and Leumit require 12 months of membership. Leumit is no longer a holdout
Storage term and cost Eggs are kept up to 5 years. Without advance written notice that the woman wants storage extended, the unit may stop storing them 60 days after warning her, subject to MoH approval; non-payment triggers the same process. An annual fee applies and is usually not covered even when retrieval was funded (check the MoH tariff, taarifon)
Usage Frozen eggs can be used for IVF later, subject to the 2-living-children cap and the age-45 own-egg ceiling at thaw

Sperm Donation (trumat zera,תרומת זרע):

Aspect Details
Legal status Legal and regulated; donor insemination may not be done in a private clinic
Who can use Single women, married couples with male infertility, same-sex female couples, and a single intended mother in a surrogacy process
Coverage The kupah funds the medical treatment (insemination, monitoring, lab work). It does NOT fund the sperm: את מנת הזרע האישה רוכשת על חשבונה. A domestic vial costs several hundred shekels, an imported one can reach thousands, and several vials are often needed
Donor selection Through licensed sperm banks (bank zera,בנק זרע). Donors are anonymous; typically origin, physical characteristics, education and blood type are shared. A donor may withdraw consent and block use of his vials as long as they have not yet been used
Pending reform A public committee chaired by Prof. Shlomo Mor-Yosef (convened 2025) is examining an identified donation track. No change is in force yet

Egg Donation (trumat bitziot,תרומת ביציות):

Aspect Details
Legal status Legal and regulated under the Egg Donation Law 2010 (chok trumat bitziot,חוק תרומת ביציות)
Who can use Israeli resident women from 18 until their 54th birthday with a medical opinion that they cannot conceive from their own eggs
Recipient fee A statutory 10,000 NIS paid to the hospital, reduced to 7,000 NIS for a recipient on the income-support benefit (havtachat hachnasa) with Bituach Leumi confirmation. This fee also funds the donor's compensation. Beyond it the kupah funds the treatment as part of the basket
Matching Same religion, not relatives, donor not married, unless an exceptions committee (vaadat charigim) approves
In surrogacy A surrogate may never also be the egg donor. A single intended mother may not use a donated egg for surrogacy

Surrogacy (pundakaut,פונדקאות): legal under the Embryo Carrying Agreements Law 1996, with at least one intended parent genetically linked to the child; couples and single individuals are eligible since the January 2022 reform. The surrogate must be an Israeli resident, at least 22 and not yet 39 at approval, who has given birth before and is not a relative. There is no official cost tariff. references/surrogacy-law-guide.md has the committee, costs, warnings about surrogacy abroad and the full process.

Posthumous sperm use and embryo disputes: there is no statute. The state acts under the 2003 Attorney General directive, use of the sperm always requires Family Court approval, and embryo disputes after separation turn on the Nahmani case. references/posthumous-and-embryo-law.md has retrieval timing, the wartime rule, who may apply, contacts, embryo storage terms and dispute rules; consult it for any posthumous-use or embryo-disposition question.

Step 6: Male-Factor Infertility and Fertility Preservation

Roughly one in three couples at a fertility unit has a male-factor component, and the skill's coverage rules treat it differently from female-factor treatment.

Topic Details
Workup Semen analysis (semen analysis), usually repeated, then a urologist or andrologist. The woman's kupah funds the couple's treatment, but treatment of the man's sperm is funded by HIS kupah. Sperm washing and selection (sperm enhancement) precedes IUI or IVF, and ICSI is basket-funded when medically indicated
Azoospermia No sperm in the ejaculate is investigated at a fertility unit; surgical retrieval from the testis (TESE, micro-TESE) belongs to that pathway. Ask the unit and your kupah what is covered before scheduling
Sperm banking Any person may deposit and freeze their own sperm. Oncology patients get kupah funding for storage, limited to 5 years, with a set number of samples per year at no personal cost
Before gonadotoxic treatment Raise preservation BEFORE chemotherapy or radiation starts; the window closes once treatment begins. The oncology social worker is the fastest route for the referral
Preservation methods and minors Egg, embryo and ovarian tissue freezing (ovarian freezing) are listed basket preservation methods, and preservation for girls and adolescents facing fertility-damaging treatment is explicitly within the funded indication. For pre-pubertal boys, ask the treating oncology unit what it offers; do not assume coverage

Step 7: Work Rights and Bituach Leumi During Treatment

Right Details
Absence, women Under the Women's Employment Regulations (Absence for Fertility and IVF Treatments) 1990, absence is charged against accrued sick days: up to 4 treatment series a year of up to 16 days each on a 5-day week (64 days a year), or 20 days each on a 6-day week (80 a year)
State-employed women Since 01.08.2018, 2 extra days per series: 18 per series and up to 72 days a year (5-day week), 22 per series and up to 88 (6-day week)
Absence, men 12 days a year against accrued sick days for fertility treatment he is undergoing
Partial-day absence Up to 40 hours a year (pro-rated for part-time) may be taken as single hours rather than whole days, and those hours are paid from the first hour
Sick pay for whole days Nothing on day 1, 50% on days 2 and 3, full pay from day 4. For men the full-pay tier runs from day 4 through day 12, and further days that year are unpaid
Anti-dismissal protection No dismissal or pay cut during the absence and for the 150 days that follow, without a Ministry of Labor permit
Reserve duty (miluim) A reservist in fertility treatment is released from active reserve service for at least: clinic-based treatment, 2 days before the expected ovulation to 3 days after (5 days); IVF, 3 days before to 7 days after (10 days); IVF with surgical sperm retrieval, 3 days before the surgery to 3 months after. Apply on Form 58 in the IDF reserves personal area with a medical certificate giving the expected ovulation date. It does not apply to an emergency call-up (Tzav 8). A reservist whose partner is in treatment may also be released, case by case
Pregnancy-preservation benefit Fertility treatment itself carries no Bituach Leumi benefit, only sick days. Once pregnant, pregnancy supervision (pregnancy supervision) pays a benefit for a doctor-certified absence of at least 30 consecutive days (14 for a subsequent period), for employed and self-employed women meeting the contribution conditions. IVF pregnancies and OHSS are common triggers
Maternity benefit Dmei leda eligibility does not depend on how the pregnancy was conceived

These are statutory rights and do not depend on which kupat cholim or SHABAN plan you hold.

Step 8: Costs and Emotional Support

Item Covered by Health Basket Out-of-Pocket
IVF or IUI cycle (first 2 living children) Yes Small copays
IVF cycle (third child, no SHABAN) No Market estimate roughly 15,000 to 25,000 NIS; get a written quote
Donor sperm vial No Several hundred NIS domestic, up to thousands imported
Fertility medications Yes Your kupah's standard prescription copay; confirm premium drugs per item
PGT (BRCA/Lynch or an approved indication) Yes, capped at 2 pregnancies ending in birth Unapproved indications are self-pay
Egg freezing (medical indication) Yes, per indication Annual storage fee
Egg freezing (elective) No 3,500 NIS per cycle on a subsidizing SHABAN plan, otherwise full price plus storage
Egg donation (recipient) Yes Statutory 10,000 NIS, or 7,000 NIS on income support
Surrogacy Mostly no Large and highly variable, no official tariff (Step 5)
Private fertility clinic (full cycle) No (unless a SHABAN agreement) Market estimate roughly 20,000 to 40,000 NIS

Support: every licensed fertility unit has a social worker at no extra cost, and kupot run free support groups. Amutat Chen (Chen Association for Fertility and Life), 03-5050345, offers counseling and peer groups. Kol HaBriut, the MoH information line, is *5400. For halachic guidance there are the Puah Institute, Bonei Olam and Keren Gefen, and Maccabi publishes an entitlement to a supervisor for fertility treatment (supervisor of fertility treatments).

Examples

Example 1: Starting IVF for the First Time

User says: "My wife and I have been trying with no success. How do we start IVF through our kupat cholim?"

Actions:

  1. See your gynecologist or family doctor. A common referral threshold is 12 months of trying, or 6 months if the woman is over 35, and sooner where there is a known problem
  2. Get a referral to a fertility specialist (rofe poryut). Both partners are evaluated: hormonal blood panel and ultrasound for the woman, semen analysis for the man
  3. If male-factor is found, his sperm treatment is funded by HIS kupah while the couple's treatment is funded by hers
  4. Get the kupah fertility committee's approval and a Form 17 to a licensed IVF unit
  5. Treatment is covered under the health basket for the first 2 living children

Result: The couple enters the pathway through their kupat cholim, with the medical costs of IVF covered.

Example 2: Coverage for a Third Child

User says: "We already have two children from IVF. We want a third. Is IVF still covered?"

Actions:

  1. Basic coverage ends after 2 living children from the current relationship, and twins from one birth already exhaust it
  2. Check your SHABAN policy for extra IVF cycles, and confirm the terms and qualification period with your kupah
  3. If SHABAN does not cover it, budget for a self-paid cycle and get a written quote from the unit
  4. If your children are from a previous relationship, say so: entitlement attaches to the current relationship

Result: The couple determines their SHABAN coverage and plans accordingly.

Example 3: Single Woman Considering Egg Freezing

User says: "I'm 33 and single. I want to freeze my eggs. Is this covered?"

Actions:

  1. Elective egg freezing is NOT in the basic basket at any age. It is permitted from 30 until the 41st birthday at your own expense, capped by MoH circular 1/2011 at 6 retrievals and 25 eggs (ages 30 to 35) or 35 eggs (ages 36 to 40)
  2. Medical egg freezing IS funded with a documented indication (cancer treatment, increased risk of premature ovarian failure up to 39, women facing prophylactic ovarian surgery)
  3. All four kupot now subsidize elective freezing through SHABAN at 3,500 NIS per cycle, on different age bands: Clalit Platinum 30 to 37, Maccabi Sheli 31 to 38, Meuhedet Si (no published age band), Leumit Zahav 30 to 37. Maccabi and Leumit require 12 months of membership first
  4. Age 33 falls inside the Clalit, Maccabi and Leumit bands (confirm Meuhedet), so compare cycle counts and egg caps rather than assuming only one kupah offers it
  5. Plan for the 5-year storage term and the written renewal notice it requires

Result: She compares her kupah's terms against the others, and knows the procedure is self-funded but subsidized.

Example 4: Exploring Surrogacy in Israel

User says: "We're a couple considering surrogacy. Is it legal in Israel and how does it work?"

Actions:

  1. It is legal under the Embryo Carrying Agreements Law 1996, with at least one intended parent genetically linked to the child. Couples and single individuals are both eligible since the January 2022 reform
  2. You apply to the approval committee with the required forms and medical, legal and psychological opinions
  3. The surrogate must be an Israeli resident, at least 22 and not yet 39, who has given birth before and is not a relative, and she can never be the egg donor
  4. Costs are large and variable with no official tariff, so get written quotes and read the Ministry of Health warnings about surrogacy abroad
  5. Consult a lawyer specializing in surrogacy law before signing

Result: The couple begins the application with realistic expectations about cost and timeline.

Gotchas

  • The limit is living children (2), not cycles, but there IS a cycle rule from 42: no more than 3 consecutive cycles that did not reach embryo transfer (any transfer resets the count), plus a mandatory team review after 4 consecutive no-transfer cycles or 8 cycles with no clinical pregnancy (Circular 6/2014). Agents say "no cycle limit" flatly and get this wrong.
  • For a woman starting at 40, give the rules as one timeline: own-egg funding runs until her 45th birthday, the Circular 6/2014 cycle rule applies from 42, elective freezing (self-pay) is permitted only until her 41st birthday, and the Clalit, Maccabi and Leumit SHABAN bands all end before 40, while Meuhedet publishes no age band, so ask Meuhedet. Scattered across answers, the rules are hard to see as one picture.
  • Twins from one funded birth exhaust the whole entitlement. The statute counts children, not pregnancies.
  • The age ceiling is layered: 45 with the woman's own eggs, 54 with a donated egg.
  • The sperm vial is NOT funded. The kupah pays the insemination and lab work; the woman buys the vial herself. This hits single women and lesbian couples hardest.
  • Egg-donation recipients pay a statutory 10,000 NIS fee (7,000 NIS on income support), not "minimal copays".
  • Elective egg freezing is NOT in the basic basket, but the 25-egg and 35-egg caps ARE Ministry of Health limits (Circular 1/2011) on that self-funded procedure, not private SHABAN limits.
  • Leumit is no longer the holdout: all four kupot subsidize elective freezing, on different age bands (Clalit 30-37, Maccabi 31-38, Leumit 30-37; Meuhedet publishes no age band).
  • Frozen eggs are kept up to 5 years, renewable only on advance written request; the unit may stop storing them 60 days after notice with MoH approval. Embryos get 5 free years plus 5 more for a fee.
  • Reciprocal IVF is illegal in Israel, which matters for female same-sex couples who assume otherwise.
  • Men get 12 absence days a year, not "shorter periods". Women get 64 or 80, state employees 72 or 88, plus 40 partial-day hours paid from the first hour.
  • The 2026 PGT entitlement for BRCA/Lynch carriers is capped at 2 pregnancies ending in birth.
  • The complaint route is not the kupah's own ombudsman but the Public Complaints Commissioner for the National Health Insurance Law at the Ministry of Health, which can determine the kupah owed the service and, in appropriate cases, get the privately paid cost reimbursed.
  • Posthumous sperm retrieval follows the 2003 Attorney General directive plus a wartime rule letting bereaved parents request retrieval without a court application. Agents omit this entirely.

Troubleshooting

Error: "My kupat cholim says I've used up my IVF coverage but I don't have 2 living children"

Cause: Coverage is based on living children, not cycles, so ask which rule is being applied; it may be a counting or administrative issue. Two real rules can also be behind it: twins from one funded birth exhaust the entitlement, and from 42 you may not run more than 3 consecutive cycles that failed to reach embryo transfer (any transfer resets the count). The mandatory team review after 4 consecutive no-transfer cycles or 8 cycles without clinical pregnancy can also pause treatment. Solution: Ask the kupah in writing which rule it is applying. If you believe the rule was applied wrongly, the route is a complaint with the Public Complaints Commissioner for the National Health Insurance Law at the Ministry of Health, attaching medical documents, correspondence and receipts. The commissioner can determine the kupah owed you the service, in appropriate cases the cost of a privately purchased service is reimbursed, and its position supports a claim in the Regional Labour Court.

Error: "I was told I'm too old for IVF coverage at age 46"

Cause: The own-egg ceiling really is 45 (עד יום הולדת 45), so the published own-egg ceiling does apply at 46, but the staff member may have stopped there. Funded treatment with a DONATED egg runs until the 54th birthday for Israeli-resident women with a medical opinion that they cannot conceive from their own eggs. Solution: Ask the fertility unit to open the egg-donation track: a written medical opinion, the official application form to the responsible physician, entry into the Ministry of Health database, and matching. Budget the statutory 10,000 NIS recipient fee (7,000 NIS on income support); the rest is funded by your kupah. If the kupah refuses the donation track itself, complain to the National Health Insurance commissioner.

Error: "I don't understand the difference between basic coverage and SHABAN for fertility"

Cause: Basic basket coverage and SHABAN overlap confusingly on fertility. Solution: Basic coverage is your legal entitlement with no extra insurance: IVF for the first 2 living children, own eggs to 45 and donated eggs to 54, plus IUI, ICSI, medically indicated egg freezing and the funded PGT indications. It does NOT cover the donor sperm vial, elective egg freezing, or a third child. SHABAN is optional paid insurance adding cycles for a third child, subsidized elective freezing at 3,500 NIS per cycle, choice of doctor, shorter waits and egg-donation extras, subject to a qualification period of up to 12 months. Ask your kupah for its current fertility chapter in writing, since these benefits changed in 2026.
