The Diagnostic Technique of Governing the Exterior to Infer the Interior

R — Original Text (Reading)

For what is distant, govern the exterior to infer the interior; for what is near, govern the interior to infer the exterior. This is called the ultimate of yin and yang, the covering of heaven and earth. Observe its external correspondences to know its internal organs, and then one knows what the disease is. Combine and examine them, feel and verify them, see and obtain them—like clear water and a bright mirror that do not lose their forms.

— Yellow Emperor/Qi Bo, Wai Tuan Chapter 45, Ben Zang Chapter 47

I — Methodological Skeleton (Interpretation)

Governing the exterior to infer the interior is a structured reasoning framework for inferring internal states that cannot be directly observed from observable external signals.

Its core operation is divided into four steps:

  1. Establish mapping: Determine the correspondence between external observable signals and internal hidden states. Facial complexion corresponds to the five zang organs, pulse manifestations correspond to qi and blood—each external signal is a "proxy indicator" of some internal state.
  1. Multi-signal collection: One cannot look at only one indicator. The Lingshu emphasizes "combine and examine them, feel and verify them"—facial complexion + pulse manifestations + body posture + forearm skin, cross-validated through multiple signals, similar to "multi-camera surveillance."

  2. Difference comparison: Precisely locate the problem by comparing signal differences at different locations (such as left vs right, upper vs lower). "Solitary heat," "solitary cold," "solitary largeness," "solitary smallness"—abnormalities are always discovered from differences.

  3. Infer the interior: Infer internal states from patterns of external signals, similar to "clear water and a bright mirror that do not lose their forms"—the clearer the signals, the more accurate the inference.

A1 — Applications in the Text (Past Application)

Case 1: Judging the state of the zang-fu organs from facial color and luster

  • Problem: How can one know the state of the internal organs without opening the body?
  • Use of the methodology: "A red complexion with small texture means the heart is small; with coarse texture, the heart is large. A white complexion with small texture means the lungs are small..." Facial complexion + skin texture = size of the five zang organs. "Observe its external correspondences to know its internal organs, and then one knows what the disease is."
  • Conclusion: The states of the five zang organs are necessarily manifested through external features such as facial complexion, skin, and body posture; there exists an interpretable mapping relationship.
  • Result: A systematic diagnostics of "knowing the interior from the exterior" was established, allowing assessment of internal states without invasive examination.

Case 2: Judging the nature of disease from cold and heat of the forearm skin

  • Problem: How can one quickly judge the cold or heat nature of a disease?
  • Use of the methodology: "If the forearm skin is blazing hot and the Renying pulse is large, there should originally be blood"—burning hot skin on the forearm + a large neck pulse = blood heat. "If the palm is hot, the abdomen is hot; if the palm is cold, the abdomen is cold"—palm temperature = abdominal cold and heat.
  • Conclusion: Signs at the distal end (the arm) can reflect the state of the core (the abdomen); there exists a remote mapping.
  • Result: Internal cold and heat can be judged through simple palpation, without complex instruments.

A2 — Trigger Scenarios (Future Trigger) ★

  1. Black-box inference: The need to understand the internal state of some opaque system/person/organization, only through external signals.
  2. Indirect assessment: The target variable cannot be measured directly, and proxy indicators need to be found.
  3. Pattern recognition: Observe a set of external manifestations and infer the underlying cause or mechanism.

Linguistic signals

  • "How can you tell what is going on inside from the outside"
  • "You cannot see the inside, so you can only judge from the outside"
  • "What do these manifestations indicate"
  • "How to judge YY through XX"

Distinction from adjacent skills

  • With four-seas-regulation difference: inferring the internal from the external is adiagnostic method(how to look), while Four Seas regulation is aregulatory strategy(how to regulate). First use inferring the internal from the external to diagnose, then use Four Seas regulation to determine which hub to regulate.

E — Executable steps (Execution)

  1. List observable signals

    • List all directly observable external signals, and evaluate the information value and reliability of each signal.
    • Completion criteria: list at least 3 observable signals of different types.
  2. Establish signal-state mapping

    • For each signal, establish a correspondence with the target internal state (positive correlation/negative correlation/conditional correlation).
    • Completion criteria: the mapping relationships are clear, not arbitrary associations, and are supported by logic or experience.
  3. Cross-validate multiple signals

    • Compare the inference results of different signals; if they are consistent, confidence is strengthened; if they contradict, the contradiction needs to be explained.
    • Completion criteria: provide a comprehensive judgment and mark signal consistency and contradiction points.
  4. Form an inference and verify it

    • Synthesize all signals to form an inference about the internal state, and set a verification method (if it can be verified).
    • Completion criteria: the inference has a clear basis and a verification path.

B — Boundary (Boundary) ★

Do not use in the following situations

  • Can be directly observed: If the internal state can be directly measured (such as opening the chassis to look at the hardware), indirect inference is not needed.
  • Overinterpretation of a single signal: One indicator cannot determine everything; only cross-referencing multiple signals through "参伍合参" is meaningful.

Failure modes

  • False positive: External signals mislead (such as facial redness not necessarily being a heat pattern), requiring multi-signal verification.
  • Outdated mapping: The signal-state mapping relationship may become invalid as the environment changes, requiring regular calibration.

The author's blind spots

  • The external-internal mapping in the Lingshu is based on empirical induction, and modern scenarios require verifying whether the mapping relationship holds. Themode of thinkingof inferring the internal from the external (inferring the true value from a proxy) is universal, but the specific mapping rules need to be re-established in each respective field.
